I acknowledge that the tattoo design, placement, and size have been discussed and agreed upon with the tattoo artist, and I am satisfied with the final design.

I hereby release the tattoo artist, studio, and their respective employees, agents, and representatives from any and all liability arising out of or in connection with the tattooing process, including but not limited to any injury or any infection.

I grant the studio the authority to share photographs of my tattoo or the tattoo procedure on social media or websites.
